Do I need a prescription from my doctor to get DME from Affinity?+

For most equipment categories — CPAP, oxygen, mobility, hospital beds — yes, your prescribing physician needs to provide a Standard Written Order and supporting documentation. You don't have to chase that paperwork yourself. We work with your provider's office directly to gather what's needed while we verify your insurance coverage in parallel.

How do I check if my insurance covers the equipment I need?+

Submit three fields through our coverage check (member ID, date of birth, equipment type). We run the actual 270/271 eligibility transaction with your payer — Medicare, Medicare Advantage, Medicaid, or commercial — and call you back with a plain-English answer, usually the same business day. There's no charge for the check and no commitment to order.

Which states do you serve?+

We are state-licensed in Arizona for full DMEPOS supply, and we ship Medicare mail-order categories — CPAP supplies, continuous glucose monitors, and select other items — nationwide. Minnesota state Medicaid enrollment is in progress. Check the locations page for the current coverage map.

Do you accept Medicare Advantage and Medicaid plans?+

Yes to both. We verify benefits across most Medicare Advantage plans (Aetna, Humana, UnitedHealthcare, Blue Medicare, regional MA plans) and AHCCCS (Arizona Medicaid) is live today. Dual-eligible coverage (Medicare + Medicaid) is supported across the board. Because each plan has its own contracted-supplier network, we always verify your specific plan before ordering.

How long does it take to get my equipment after I qualify?+

Standard turnaround on mail-order categories (CPAP supplies, CGM, diabetes supplies) is typically 5–7 business days from documentation complete to delivery. Initial-setup equipment (new CPAP, oxygen concentrator, power mobility) takes longer because of the documentation chain — usually 7–14 business days depending on payer and how quickly the prescribing office returns paperwork. We tell you the realistic timeline upfront, not a marketing one.

What if my insurance doesn't cover what I need?+

We tell you in writing before anything ships. You can pay out-of-pocket with full price transparency, work with your physician on alternative documentation, or have us refer you to a different supplier that may have your plan contracted. We don't ship first and bill later — that's how surprise bills happen, and the whole product is built to prevent them.
